LIABILITY WAIVER, ASSUMPTION OF RISK AND INDEMNITY

By accepting, I confirm that I am the parent or legal guardian of the child listed and that I agree to the following:

I acknowledge that participation in activities at Play Café involves inherent risks, including slips, falls, collisions, and other injuries. I voluntarily accept full responsibility for all risks of injury, illness, loss, or damage to my child, myself, or our property while on the premises.

​

 

To the fullest extent permitted by South African law, I release, waive, and discharge Alchemy Kids, trading as Play Café, including its owners, directors, employees, contractors, and agents, from any and all claims, liabilities, damages, or expenses arising from any injury, illness, loss, or damage sustained while on the premises, including those arising from negligence.

​

 

I agree to indemnify and hold harmless Alchemy Kids against any claims, damages, legal costs, or expenses arising from my child’s participation in activities, failure to follow Play Café rules, or damage caused by my child to persons or property.

​

 

In the event of an emergency, I authorise Alchemy Kids staff to seek medical treatment for my child if I cannot be reached. All medical costs remain my responsibility.

​

 

Unless explicitly stated otherwise, Alchemy Kids does not provide one-on-one supervision. I remain responsible for supervising my child at all times unless I have booked and paid for an approved supervised service.

​

 

I confirm that my child is medically fit to participate and that all relevant medical, behavioural, and developmental information has been disclosed.

 

I agree to comply with all Play Café rules and staff instructions. Alchemy Kids reserves the right to refuse entry or remove any child or adult for unsafe or inappropriate behaviour without refund.

​

 

This agreement is governed by the laws of the Republic of South Africa.

​

 

I confirm that I have read, understood, and agree to the terms of this Liability Waiver, Assumption of Risk, and Indemnity Agreement.
